Waiver

Monster Mayhem Waiver & Release of Liability

Please read carefully and sign this waiver on behalf of yourself (if you are over 18) and/or any children in your care who will be using our facilities.
You must either be:

• the person listed on the booking, or
• the parent, legal guardian, or a responsible adult for the participant(s).

If you are not the parent/guardian, you confirm that you have been given permission by the parent(s)/guardian(s) to act as the “Responsible Adult” for the participant(s) and that you have authority to sign this waiver on their behalf.

Section A: Adults (18 and over)

I wish to take part in the activities organised by Monster Mayhem Limited (“the Company”), including but not limited to play area access and use of play area (“the Activities”). I agree as follows:

1. I confirm I am 18 years or older.
2. I agree to follow all safety rules provided on the Monster Mayhem website, displayed in the venue, explained in the safety briefing, and given as oral instructions before or during the session.
3. I understand socks must be worn at all times. Without socks, I cannot participate.
4. I am aware the play area has two emergency exits. In an emergency I will leave by the nearest exit (including the main entrance if safest).
5. I accept full responsibility for my own safety and belongings while participating.
6. I confirm I am in good physical health, with no medical conditions* that increase the risk of injury to myself or others.
7. To the best of my knowledge, I am not pregnant.
8. If I have any health concerns, I will notify Monster Mayhem before booking. Where necessary, Monster Mayhem may carry out a risk assessment or, if this is not possible, issue a refund. If no prior disclosure is made, Monster Mayhem reserves the right to refuse access on the day.
9. I confirm I do not exceed the maximum weight limit of 125 kg / 19 stone.

Section B: Children (under 18)

I am the parent/guardian/responsible adult of the child(ren) listed below and wish them to participate in the Activities. I agree as follows:

10. I am responsible for ensuring the child(ren) follow all safety rules provided by Monster Mayhem (website, in-venue, briefing, and instructions during the session).
11. I will ensure each child wears socks at all times.
12. I will supervise the child(ren) in my care throughout the session and accept responsibility for their safety and belongings.
13. To the best of my knowledge, the child(ren) are in good physical health, not pregnant, and have no medical conditions that would increase the risk of injury.
14. If I have concerns, I will seek medical advice and notify Monster Mayhem before booking. If no prior disclosure is made, Monster Mayhem reserves the right to refuse access where necessary.
15. I confirm the child(ren) do not exceed the maximum weight limit of 125 kg / 19 stone.

Section C: All Participants

• I understand the Activities are physically demanding and may involve jumping, sliding, climbing, speed, height, and unpredictable surfaces.
• I understand Monster Mayhem does not assess individual abilities or suitability; it is my responsibility to judge whether I/my child(ren) are able to take part safely.
• I accept that all age groups use the facilities together.
• I acknowledge the risk of serious injury or death from participation, including but not limited to: friction burns, cuts, bruising, sprains, dislocations, broken bones, or paralysis.
• I/my child(ren) participate entirely at our own risk.
• I will not hold/cradle babies or small children while using the play area.
• I will not attempt flips or extreme stunts.
• I understand Monster Mayhem does not provide direct supervision, training, or tuition.
• I accept Monster Mayhem is not liable for loss, damage, or injury except in cases of proven negligence.
• I understand CCTV is in operation for safety and security.

Safety Rules (must be followed at all times)

• Feet first down slides; check the bottom is clear; one at a time.
• No bouncing on the marked entry/exit areas.
• Socks must be worn at all times.
• No climbing on arena walls.
• No rough play, wrestling, tackling, or shoving.
• No sitting or lying on the play area.
• No food, drink, or chewing gum allowed.
• No participation under the influence of drugs or alcohol.
• Empty all pockets (keys, phones, etc.) before entry.
• No belt buckles, studs, or jewellery.
• Do not leave clothing on the play area.
• Do not participate if pregnant or injured.
• Report all injuries to staff before leaving.
• At the end of the session, collectany valuables from staff.
• If the play area (inflatables) deflate, exit calmly through the nearest exit and wait for staff clearance before re-entering.

Declaration

By signing below, I confirm that:

• I (and any child(ren) under my care) have read, understood, and agreed to the above rules and terms.
• I accept full responsibility for participation in the Activities.
• This waiver will remain on file for future visits unless revoked in writing and confirmed by Monster Mayhem.
